SMILE vs LASIK vs PRK vs ICL in Barcelona: Which Surgery and Cost
How SMILE, LASIK, PRK and ICL lenses compare in Barcelona in 2026: who each suits, recovery, published prices and the questions to ask before choosing.
The short answer
In 2026 LASIK is advertised in Barcelona from about 990 to 1,100 euros per eye, while published SMILE prices run roughly 2,000 to 3,000 euros per eye across Spain and clinics rarely publish Barcelona prices for PRK or ICL. Which procedure suits you depends on your corneas, prescription and dry eye, and only a full assessment can say.
LASIK is the most widely priced and best-documented laser eye operation in Barcelona, with advertised prices from about 990 to 1,100 euros per eye in 2026, while SMILE, PRK and implantable lenses (ICL) are chosen when LASIK is not the best fit. Published prices for the others are much thinner: Spain-wide guides put SMILE at roughly 2,000 to 3,000 euros per eye, and Barcelona clinics mostly give PRK and ICL prices on request. This guide explains what separates the four options, who each is said to suit, how recovery differs, what is published on cost, and what to ask. It is general information, not medical advice, and only a full assessment by a surgeon can say what is suitable for your eyes.

What is the difference between LASIK, SMILE, PRK and ICL?
LASIK, SMILE and PRK reshape the cornea with a laser, while ICL places a lens inside the eye. The Barcelona ophthalmology centre VERTE describes PRK and its transepithelial version (TransPRK) as flapless laser options, and TransPRK as its preferred alternative when LASIK is not viable. It describes ICL, an implanted lens, for high myopia, high hyperopia, dry eye or insufficient corneal thickness. SMILE does not appear on VERTE's LASIK page.
LASIK
LASIK is the reference option and the one with the clearest published Barcelona prices. Recovery is quick: VERTE reports functional recovery in about 24 hours. Suitability and risks are covered in the LASIK guide above.
SMILE
SMILE is a minimally invasive laser alternative that usually costs more than LASIK. In a Belgian clinic's comparison table, which we use only to describe the technique, SMILE and femto-LASIK both mean about two days off work, while PRK means about seven days and up to four to five weeks to full recovery. Not every Barcelona clinic offers SMILE, so confirm which procedures a surgeon performs regularly.
PRK and TransPRK
PRK works on the surface of the cornea without a flap, which VERTE says makes TransPRK the alternative when LASIK is not viable. The trade-off is a slower recovery. The same Belgian clinic's table lists irritation in the first 48 hours and up to four to five weeks to recover fully, against a few days for LASIK and SMILE, so ask your surgeon what to expect.
ICL (implantable lens)
ICL is generally considered when laser surgery is not suitable, for example with a high prescription, thin corneas or dry eye. VERTE lists it for those cases and suggests it as an alternative to laser for moderate to advanced dry eye. The Belgian clinic says it suits very high prescriptions from about plus 12 to minus 18, astigmatism up to 6 dioptres and a prescription stable for at least a year, and describes the lens as removable, but this is one clinic's marketing, and its own page gives two different minimum ages.
How much do SMILE, PRK and ICL cost in Barcelona?
Published Barcelona prices exist for LASIK but are scarce for the others, so treat the figures below as indicators, not quotes.
- LASIK: from about 990 euros per eye at one Barcelona clinic (Operarme, 17 October 2026) and from about 1,100 euros per eye at another (VERTE, 8 May 2026).
- SMILE: roughly 2,000 to 3,000 euros per eye across Spain (Universal Medical Travel, updated 22 June 2026). The aggregator Bookimed (13 October 2026) shows a Barcelona SMILE average of about 3,600 dollars within a range of 2,800 to 4,300 dollars, but it does not say whether that is one eye or both, and it lists only one clinic.
- PRK: from 700 euros at a Barcelona refractive surgery centre on the directory WhatClinic (page cached 4 October 2026), which does not say whether this is per eye or for both. Bookimed lists PRK from about 2,800 dollars as a package.
- UK comparison: TreatCompare (updated 29 April 2026) lists SMILE at 1,500 to 2,500 pounds per eye and TransPRK at 1,000 to 2,000 pounds, against 1,195 to 2,000 pounds for standard LASIK, so SMILE tends to cost more than LASIK there too.
- ICL: we found no published Barcelona price. The Belgian clinic's page lists from 2,500 euros per eye, or 2,900 with astigmatism, plus 160 euros for pre-operative tests, which is only a rough guide to the order of magnitude.
The sources disagree and several do not say whether a price is for one eye or two, so ask each clinic for an itemised quote per eye, naming the procedure, the laser platform and what aftercare and retreatment are included. You can sense-check what you are quoted against our cost checker.
Which one is right for me?
That depends on your corneas, prescription and eye health, and a surgeon decides after a full assessment. VERTE's assessment takes 60 to 90 minutes and includes corneal topography and tomography, pachymetry (thickness), corneal biomechanics, tear quality and a fundus exam. It says it would decline LASIK if your prescription has not been stable for a year, if your corneas are thin or irregular, if you are pregnant or breastfeeding, or if you have moderate to advanced dry eye.
- Thin or irregular corneas: ask whether PRK or TransPRK is better than LASIK for you.
- Dry eye: ask whether any laser is advisable, or whether an implantable lens is a safer route.
- High prescription: ask about ICL, and about expected results with laser at your prescription.
- Fast return to work: ask how many days off each procedure typically needs.
- Stability: if your prescription has changed in the past year, expect to be asked to wait.
What risks apply, and what does the FDA advise?
The US FDA's LASIK pages list vision loss, glare and halos affecting night vision, under- or over-correction, severe dry eye that may be permanent and unknown long-term effects. It advises comparing procedures, devices and surgeons, not basing the decision only on cost, and being wary of money-back or package deals. Those pages are dated 2018 and concern LASIK specifically, so ask your surgeon for current figures on the procedure you are considering.
Treating both eyes in one session is riskier than treating them separately, and monovision, correcting one eye for distance and one for near, can reduce depth perception, according to the FDA. If a clinic proposes either, ask why. See also our guide to eye tests, opticians and ophthalmologists in Barcelona for the first-step assessment.
What should I ask before booking?
- Which procedure do you recommend for my eyes, and why not the others?
- Which laser or lens platform will be used?
- What exactly is included in the price per eye, and what is extra?
- How long is aftercare included, and what are retreatment terms?
- Who is the surgeon, and will they explain the risks and consent form in English?
- How many of these procedures do you perform a year, and what are your complication rates?

Common questions
What is the difference between SMILE and LASIK?
Both reshape the cornea with a laser, and SMILE is described as the more minimally invasive option. In published prices SMILE usually costs more: roughly 2,000 to 3,000 euros per eye across Spain, against about 990 to 1,100 euros per eye advertised for LASIK in Barcelona.
How much does PRK cost in Barcelona?
A Barcelona centre is listed from 700 euros on the directory WhatClinic, but the listing does not say whether that is per eye or for both. Barcelona clinics often give PRK prices only after an assessment, so ask for a written per-eye quote.
How much do ICL lenses cost in Barcelona?
We found no published Barcelona price for ICL. Clinics generally quote after assessment, and one European clinic's page lists from 2,500 euros per eye, which is only a rough indicator.
Which is better for thin corneas or dry eye?
That is a surgeon's decision. A Barcelona clinic says it would decline LASIK for thin or irregular corneas and for moderate to advanced dry eye, and offers TransPRK or an implantable lens as alternatives.
How long is recovery after SMILE, LASIK and PRK?
One Belgian clinic's table lists about two days off work for SMILE and femto-LASIK and about seven days for PRK, with up to four to five weeks to full recovery after PRK. Your surgeon will give your own timeline.
